MPS requires all parent volunteers going on school field trips to submit a Background Check form. This background check is good for two years from the date it was approved. To check your volunteer status, please contact the classroom teacher or Family Liaison Lori Smith at firstname.lastname@example.org. Background checks can take up to two weeks to process, although many times it can be completed earlier. Background checks will take longer in the beginning of the school year.

All volunteers in MPS buildings must sign the district form stating you certify that you are either vaccinated for COVID or will submit to weekly testing. Forms can be found in the office or online. Please show office staff your signed form the first time you volunteer.
